Value-based care is here: How health IT can help

Value-based healthcare services require medical systems to go beyond their four walls, interact with the people they serve, and achieve cost, quality, and risk visibility across multiple settings and data sources.
While value-based healthcare is almost everyone's attention, it's hard to do it successfully. Historically, it has proven difficult to determine the quality of patient care. Value-based care provides a new perspective through which patient care can be viewed and laid the foundation for the future of all stakeholders to collaborate to achieve seamless care. Many health care leaders question how to begin their organizational transformation. However, regardless of the health system, one thing is certain: a value-based care journey begins with providing timely and actionable data when managing what we measure.
, EHR's limitations in value-based care
The quality of the higher than the quantity needs to understand the current medical environment. A recent survey reported that while most medical institutions indicated that they are preparing to switch to a new payment system, half of the respondents admit that their value-based technology infrastructure is not strong enough. In the age of value-based healthcare, healthcare providers need sophisticated patients who can accurately report patient data and monitor multiple providers. However, the relative lack of true interoperability in healthcare means that it is often difficult for doctors to aggregate correct patient data at the point of care.

This challenge is partly due to the large-scale implementation of electronic health records (EHRs) and the meaningful use under the “Affordable Care Act”. This has driven the adoption of electronic order entry and enterprise EHR, which requires the health system to reorganize the workflow to work with its EHR. It does not simplify care coordination as expected, but often keeps in touch with inpatient and outpatient systems that do not communicate. Health systems now typically operate under many different EHRs in their network, and patient data is scattered among these EHRs. Therefore, doctors may not be able to provide quick, informed care decisions if the correct patient data is not easily accessible.
